Full payment is due at the time of booking online.
Appointment Cancellation
Please reschedule your appointment if you are experiencing cold or flu-like symptoms. Last-minute cancellation fees due to illness will be waived as long as you contact me before your appointment. Cancellations with at least 24 hours notice will not be charged. Cancellations with less than 24 hours notice will warrant payment for the scheduled appointment. This includes a “no-call” or “no-show”.
Client Health Intake & HIPAA
If this is your first appointment at Stillpoint, you will be asked to complete a Client Heath Intake which reviews your current health, health history and daily activities. Also included in this intake is a standardized HIPAA Privacy Policy Notice. Please take a moment to complete this form prior to your first appointment.
Insurance
I am no longer accepting insurance. However, at your request, I will provide you with an itemized invoice with appropriate billing code for insurance. You can submit this invoice along with a prescription from your provider for insurance reimbursement associated with Worker’s Compensation or Personal Injury Protection (automobile accident).
